Dear Patient:

Welcome to our practice.  We are very pleased that you have selected us for your medical care.  There are forms available that you may download and print to fill out in advance of your appointment to assist our staff and your treating provider in making sure that we have all the information necessary to provide you with quality care and treatment.  Please fill out all the forms completely in black ink.  If you have any questions or problems filling out the forms, do not hesitate to call so that we may assist you.  Please bring completed forms to your appointment.

If you have been treated by another physician or hospital then you may want to request copies of pertinent medical records in advance of your appointment.  The records may be given directly to you or they can be mailed to our office.  You can download the medical release form by clicking on the Patient Forms link above.
